Yeah, I couldn't figure that out for the longest time.
You have to apply for an entertainer's account.

There's the link. All you have to do is give them a description of what your show will be like. You will get a message 1 or 2 days later telling you that you have been accepted as an entertainer. Then you go to "edit profile" and there will be more options to edit your profile with! From there, you click "upload banner" and you will be all set. :D
